National Clinical Services Privacy Policy
The purpose of this policy is to outline how National Clinical Services (NCS) complies with confidentiality and privacy obligations. As an organisation NCS’s principal is and always will be the health of patients who visit our clinics. A high level of trust and confidential is required to ensure the confidence of the patients we serve.
Patients will be assured that:

Their information collected will be retained in our records and may be reconfirmed again at any time to ensure it is correct and up to date.
SECTION 1 – WHAT DO WE DO WITH YOUR INFORMATION?
When you get referred to our clinic, or come to purchase something from our clinic, as part of our clinical process, we will collect personal information about you, which includes, but is not limited to, Name, Date of Birth, Residential Address, Contact Number, Driver’s License, Email Address and Medicare Details.
This information may be used as outlined in Section 4, to assist a Third-Party Service Provider.
SECTION 2 – CONSENT
How do you get my consent?
When you provide us with your personal information, verbally or in writing, we imply that you consent to our collecting it and can use this for assisting your clinician.
If we ask for your personal information for a secondary reason, like marketing, we will either ask you directly for your expressed consent or provide you with an opportunity to decline.
How do I withdraw my consent?
You may withdraw your consent at any time for us to contact you regarding a secondary reason, such as marketing etc. but any information we collect from you which will assist your clinic regarding any diagnosis made by our clinician or specialist will be kept on your file. If you would like us to deactivate your file with us, this may be done.

SECTION 3 – DISCLOSURE
We may disclose your personal information if we are required to by law or to do so if you violate our Terms of Service. We have the right to discuss your medical information with your GP, Referring Doctor, Hospital, third-party services (as outlined in Section 4) and the RMS if required (Section 6).
SECTION 4 - THIRD PARTY SERVICES
In general, the third-party providers used by us will collect, use, and disclose your information to the extent necessary to allow them to perform the services they provide to us.
However, certain third-party service providers, such as a reporting Sleep & Respiratory Specialists have their own Privacy Policies in respect to the information, we are required to provide to them regarding your information.
For these providers, we recommend that you read their Privacy Policies so you can understand the manner in which your personal information will be handled by these providers.
In particular, remember that certain providers may be located in a different jurisdiction that either you or us. So, if you choose to proceed with us and a third-party service provider, then your information may become subject to the laws of the jurisdiction(s) in which that service provider or its facilities are located.
SECTION 5 – SECURITY
To protect your personal information, we take reasonable precautions and follow industry best practices to make sure it is not inappropriately lost, misused, accessed, disclosed, altered, or destroyed.
If you provide us with information such as credit card information, this will be kept on your accounting file.
SECTION 6 – ROADS AND MARITIME SERVICS (RMS)
As we are a medical clinic, your personal information may be shared with the RMS at any time if we have concerns regarding your license with associated diagnosis made by either our clinicians or a third-party specialist. We may ask for your drivers as outlined in Section 1 of this policy.
SECTION 8 – KEEPING RECORDS
NCS will keep all patient records for a minimum of 7 years from the last date of entry.
SECTION 7 – CHANGES TO THE PRIVACY POLICY
We reserve the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take effect immediately. If we make material changes to this policy, with what information we collect, how we use it, and under what circumstances, if any and/ or disclose it, this may be done without notice.
If our store is acquired or merged with another company, your information may be transferred to the new owners so that we may continue to sell products to you.
