The Referral:

*requirements: A Referral MUST be signed and dated by the Referring GP/Specialist, and include all relevant patient information.

The Appointment:
The client will attend an initial clinical consultation with a qualified clinician. Following an assessment of the referral and the client’s presenting concerns, the clinician will determine the most appropriate course of assessment.
This may include completing diagnostic testing onsite, providing equipment or testing for completion at home, or conducting a general clinical consultation to assess the client’s needs and determine any appropriate follow-up.

The Results:
Following completion of the assessment or diagnostic testing, the results will either be discussed directly with the client or forwarded to the referring GP, depending on the nature of the referral and clinical requirements.
A copy of the completed results will also be provided via email. If you have not received the results, please contact our office on 1300 301 639 or email info@ncservices.com.au.
When requesting a copy of results, please ensure you have the following information available to assist us in locating the correct record:

This information is required to ensure client records are accurately identified and results are securely provided to the appropriate recipient.
